1 - Failure to fill up the form means that when you arrive at our salon we cannot get you to fill up the form on your time because we won't have time to finish your services booked and paid for.

2 - The form is so you can tell us things about you that relate to the service you are getting, meaning that we will be able to take care of you based on the questions asked.

3 - Failure to put solid answers means we will still ask you the important questions as we start and act on what's given, but you could say anything and if you have any reaction you wouldn't have anything to use against us apart from your word and we would have everything to show that we tried to ask for solid answers to be written on paper for us to know about you.

So it all comes down to duty of care, we do everything we can to take care of you and give you the best experience we can, that's why we want a form filled before you arrive so that we are ready for you.
